The purpose of the present study was to investigate whether mechanical efficiency (ME) relates to medical scooters propulsion capacity and wheelchair performance tasks during and after rehabilitation of people with a spinal cord injury (SCI). Eighty participants with a SCI were tested during rehabilitation (3 x) and 1 year after discharge. Two 3-minute submaximal exercise blocks, a maximal medicare power wheelchairs exercise test, and four wheelchair performance tasks were performed. ME, peak power output (PO (peak)), the sum of the performance times of a 15-m sprint and figure-of-eight, and the heart rate reserve (%HRR) during 10 s of wheelchair propulsion on a 3 % and 6 % slope were calculated. The relationship between ME and PO (peak), %HRR and performance time was tested with a multilevel regression analysis. ME showed a significant relationship with PO (peak) (p = 0.002). A 1 % higher ME related to a 1.6 - 2.2 W higher PO (peak). ME of exercise block 2 was related to the sum of the performance times of a 15-m sprint and figure-of-eight; the tests were performed faster by participants with a higher ME. No relationship was found between ME and %HRR during wheelchair propulsion on a slope. ME showed a significant effect on wheelchair propulsion capacity and wheelchair performance time during and 1 yr after SCI rehabilitation although the isolated effect of ME is small.

Influence of hand cycling on physical capacity in the rehabilitation of persons with a spinal cord injury: a longitudinal cohort study
OBJECTIVE: To investigate the influence of handicap scooters on outcome measures of physical capacity during and after rehabilitation in persons with paraplegia and tetraplegia in The Netherlands. DESIGN: A longitudinal cohort study with measurement moments at the start (t1) and end (t2) of clinical rehabilitation and 1 year after discharge (t3). Hand cycle use was assessed by means of questionnaires at t2 and t3. SETTING: Eight rehabilitation centers in The Netherlands. PARTICIPANTS: Subjects (N=162) with a recent spinal cord injury. INTERVENTIONS: All subjects followed the regular rehabilitation program. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

OBJECTIVE: To determine how research regarding medicare power wheelchairs and medical scooters and seating selection and configuration could be applied to the pediatric population with spinal cord injury (SCI) and dysfunction. METHODS: Extensive literature reviews were completed on (a) powered mobility options and age-appropriateness for training; (b) manual wheelchair configuration, pushrim biomechanics, and propulsion ergonomics; and (c) cushion selection and use of pressure-mapping devices. The findings in the literature review then were compared with clinical observations from Shriners Hospitals for Children, Philadelphia’s Seating and Wheelchair Clinic. RESULTS/DISCUSSION: A significant amount of research is published on propulsion ergonomics and pushrim biomechanics in adults with SCI. However, this literature review noted that there was limited research available on cushion selection and power mobility configuration. Many of the conclusions drawn from these publications are applied to pediatric patients and used during the decision-making process of wheelchair and cushion selection. However, it is critical to tailor the assessment to the pediatric population to meet their medical and functional needs. CONCLUSION: Although some of the findings can be correlated to the pediatric population, there is a lack of research on wheelchair and seating selection and configuration as it specifically relates to the young person with SCI and dysfunction. Future studies need to be completed.
